Do you have a runny nose, sore throat, new cough or breathing problems, fever, fatigue, muscle aches?

Have you traveled in the past 14 days or been in close contact with anyone that has?

If you’ve answered yes to any of the above, please cancel your appointment and reschedule for a later date; there will be no fees during this time for late cancellations due to illness.
